If you wish to make a referral for occupational therapy
Please visit the CYPOT website and click on the Referral Information and Referral Criteria sections where you will find the Referral Form and Request More Help Form. All referrals are received by the Children and Young People’s Occupational Therapy Team (CYPOT).

If you need more help or would like to follow up on a referral
If you still require support with your referral, please call the Administration Hub: 020 7266 8777. This phone line is open from 8am to 6pm, Monday to Friday. Please have your child’s NHS ID available.
