Information Submitted via the Contact Form
The Contact Us form will be received following submission to clch.otsupporthub@nhs.net to be actioned by the CLCH CYPOT Admin team, as follows:
- If you use our Contact Us form from the OT support Hub, we will process your request within 10 working days.
- If you send a request to contact a SEN OT in the Local Authority, your request will be handled securely and sent to an email with an accredited domain.
- If you are a parent or carer of an existent patient, we will upload the contact form to your patient record, and then delete your email.
- If you are not a parent or carer of an existent patient, then after actioning your request within 10 working days, we will delete your email.
All deleted emails will be removed permanently from the Trash folder at the end of the month.
Any request received will be processed according to this privacy policy.
Changes To This Privacy Policy
We reserve the right to update or change our Privacy Policy at any time and you should check this Privacy Policy periodically. Your continued use of the Service after we post any modifications to the Privacy Policy on this page will constitute your acknowledgment of the modifications and your consent to abide and be bound by the modified Privacy Policy.
